Steroid Un-Responsive Encephalopathy Associated with Autoimmune Thyroiditis (SUEAT) in Pediatric Patients.

Objective We report a series of children with encephalopathy associated thyroid antibodies who are refractory to steroid monotherapy. Background Steroid Responsive Encephalopathy autoimmune thyroiditis (SREAT) is rare condition, only few isolated cases reported in children. Marked clinical improvement following treatment steroids, hallmark SREAT. Design/Methods An IRB approved chart review was conducted on patients <18 years diagnosed encephalitis. A retrospective analysis features, diagnostic tests, response therapy and long term follow up positive for Thyroperoxidase (TPO) antibodies. Results 52 were encephalitis, 10 (19.2%) these TPO Median age at disease onset 14.5 (range 6-18 years) 1 male patient being. Mental status (90%) behavior changes (100%) most common presentations, seizures detected patient. MRI (20%) EEG (30%) abnormalities uncommon, had evidence inflammation cerebrospinal fluid (CSF). Autoimmune encephalitis paraneoplastic antibody panels negative besides 2 having concomitant Thyroglobulin (TG) All needed additional IVIG after steroids treatment, 7 (70%) received Rituximab 3 Plasmapheresis. recovered an average 4.4 up. Conclusions can be unresponsive the pediatric population. Further immune therapy, including plasmapheresis, should considered patients, even absence other para-clinical inflammation.
